CONSENT TO RECEIVE PSYCHOLOGICAL SERVICES :
This consent form explain the nature of the psychological services that you are about to receive. As consent is an ongoing process, any changes that may influence your consent will be discussed with you.
Nature of treatment: (1) Evaluation and treatment planning : Approximately 1-3 session
(2) Intervention : Depends on many factors, such as the nature of your difficulties and readiness for change
(3) Termination : Approximately 1-2 session
(4) Involves developing a "toolbox" of strategies that may be used to help you maintain your treatment gains and reduce the likelihood of relapse and / or reoccurrence.
(5) Treatment effectiveness varies from person to person.
FEES AND PAYMENT :
1. Session are approximately 45-50 minutes in length
2. as per session giving in to registration form
3. If you want to cancel your appointment please inform us by message or by one phone call
